Birth Certificate & Passport Information

Here is an outline of the steps you must take in order to obtain a Birth Certificate and Passport for your child/children. More detailed step-by-step instructions will be mailed to our intended parents during the last trimester of pregnancy.
  1. When you arrive at the hospital you will need to locate the office of the Birth Certificate Clerk. Generally, this office is open Monday through Friday from 8:00 a.m. to 4:30 p.m.
  2. At the time of birth, you must request that the Birth Certificate Clerk provide you with a birth certificate in order to obtain a passport for your baby.
  3. Once the Certificate of Live Birth is delivered to Vital Records, you must go to Vital Records and request that they provide you with a copy of the birth certificate. This service usually takes one to two days.
  4. Birth Certificates are obtained through the County Government Offices in the Health Services Department Office of Vital Records. The Birth Certificate Clerk at the hospital will be able to give you the address and phone number for the County Office near you. Passports are issued through U.S. Government Offices. If you are in the Los Angeles area, you can call the Passport Office in the Federal Building at 11000 Wilshire Boulevard, Suite 1000.

  5. Passports can be obtained once you have a valid birth certificate for your baby. For security reasons, passport offices no longer allow "walk-in" appointments. You need to call the passport office and request an appointment.
PLEASE NOTE: due to security at the Federal Building, you must arrive for your appointment on time - no more than 15-minutes early or 15-minutes late. If early, do not mill around the building. If late, do not try to keep the appointment. Call and make another appointment. Also note that you will not be the only person with an appointment at that time. A number of people will be given the same appointment time and each of you will receive a number upon arrival. Therefore, be prepared to wait at least an hour until they call you depending on the time that you arrive.
