CSP’s surrogate requirements
As the oldest US surrogacy agency – and in fact, the oldest surrogacy program in the world – the Center for Surrogate Parenting (CSP) has spent more than 40 years developing and fine-tuning our surrogate requirements. By ensuring that every applicant meets these pre-qualifications for surrogacy eligibility, we can help ensure that both the gestational surrogate and hopeful parents have a positive surrogacy experience.
At CSP, we believe that it is critical to determine whether a woman has both the physical and mental health to become a surrogate and carry a healthy pregnancy. This begins with our basic surrogacy eligibility requirements, and continues with our intensive pre-screening process that includes both psychological and medical testing.
Could I be a surrogate?
Before you apply to become a surrogate, please check to ensure that you meet the following surrogate requirements.
- Have a BMI of 31 or less
- Be between the ages of 21 and 41
- Have had at least one child who you are raising
- If receiving government assistance, must also have a source of income
- Free of medications for anxiety or depression for at least one year under doctor guidance

If you meet our requirements to become a surrogate mom, we invite you to complete our online gestational surrogate application. After we approve your application, you will hear from one of our coordinators, all of whom were once surrogates with CSP. Your coordinator will schedule a phone interview, an evaluation with a counselor, and a basic medical screening.
We will also give you access to our online portal, where you can see the progress of your application and documentation. You’ll have the full support of our experienced team every step of the way, so don’t hesitate to reach out to us with any questions.
Once your screening is complete and you have peen approved for surrogacy eligibility, our US surrogacy agency will work with you to select the hopeful parents you’d like to work with. Our team will support you through the process, from taking the medications to undergoing the embryo transfer. And, we’ll be by your side through the entire pregnancy, making sure you have seamless communication with the parents and feel comfortable and supported every step of the way.
Throughout the joyous process of bringing new life into the world, you will receive generous compensation and insurance coverage, if needed. You’ll also receive reimbursement for lost wages, a monthly allowance, childcare and travel expenses.
